Study Confirms Low-Dose-Rate Brachytherapy as the Best Option for Recurrent Prostate Cancer

A recent study published in the British Journal of Urology International confirms that low-dose-rate brachytherapy (LDR-BT) significantly improves cancer control  in men with recurrent prostate cancer compared to high-dose-rate brachytherapy (HDR-BT). This reinforces what many experts already know: LDR-BT has the highest level of evidence for treating recurrent prostate cancer, more than any other salvage treatment, including stereotactic body radiation therapy (SBRT), cryotherapy, or high-intensity focused ultrasound (HIFU).

What is Brachytherapy?

Brachytherapy is a highly targeted form of radiation therapy where tiny radioactive seeds are placed directly into the prostate. This method delivers radiation precisely to the cancer cells, while minimizing damage to healthy surrounding tissue.

There are two main types of brachytherapy:

  • Low-Dose-Rate Brachytherapy (LDR-BT): Radioactive seeds are permanently implanted in the prostate, slowly releasing radiation over time.
  • High-Dose-Rate Brachytherapy (HDR-BT): A temporary method where radiation is delivered in a short, intense session with no seeds left in place.

What Did the Study Find?

This study reviewed 31 research papers to evaluate salvage LDR-BT for men whose cancer returned after initial radiation therapy. They found that men treated with LDR-BT had a significantly higher chance of staying cancer-free  compared to those treated with HDR-BT, with an average duration of approximately 12 years!

This confirms that LDR-BT is the best-supported salvage treatment for recurrent prostate cancer, with more evidence than any other option.

Why is LDR-BT the Best Choice?

LDR-BT is the only salvage treatment for recurrent prostate cancer that has been tested in a randomized trial. Other options, like SBRT, cryotherapy, and HIFU, do not have the same level of proven effectiveness.

🔹 SBRT (Stereotactic Body Radiation Therapy) – A non-invasive radiation method, but lacks randomized trial data and long-term outcomes in the salvage setting.
🔹 Cryotherapy – Uses extreme cold to destroy cancer cells, but has higher complication risks and lower success rates than LDR-BT.
🔹 HIFU (High-Intensity Focused Ultrasound) – A newer technology still in experimental stages, with no randomized trial evidence in the salvage setting.
